- Schedule patient referrals to specialists, diagnostic facilities, and other healthcare providers, based on clinic protocols and patient needs.
- Verify patient insurance coverage and ensure that all necessary authorization and documentation are complete before scheduling referrals.
- Coordinate with external providers to confirm appointments, procedures, and any specific patient preparation requirements.
- Work closely with primary care providers and clinical teams to determine the most appropriate referral options for patients based on medical needs and clinic resources.
- Review patient records and consult with providers to gather all relevant information for each referral.
- Communicate directly with patients to explain the referral process, answer questions, and address any concerns about the referral.
- Assist patients with logistical aspects of their referrals, such as arranging transportation, understanding preparation instructions, and gathering any required documentation.
- Follow up with patients to ensure they have received appointment details and understand their next steps.
- Serve as a point of contact for patients throughout the referral process, providing ongoing support and guidance as needed.
- Document all referral activities, including appointment details, patient communications, and follow-up notes, accurately and promptly in the electronic health record.
- Track and document referral outcomes, ensuring that results and specialist reports are received and filed in the patient’s chart for provider review.
- Maintain a referral tracking system to monitor the status of all active referrals and follow up on any delayed or incomplete referrals to ensure timely care.
- Participate in quality improvement initiatives related to referral management and patient continuity of care.
- Suggest and implement process improvements to enhance efficiency and patient satisfaction within the referral process.
- Stay informed of any changes in referral processes, insurance requirements, or clinic policies, updating procedures accordingly.
